VALMER - Project Methodology

 

 

2 HMRs Recorded and classified:

 

Information from each of the HMRs will be entered into a purpose-designed database, and classified according to a modified D.O.C.U.M.E.N.T. drug-related problem classification system (14) . This published, validated system will provide information on the type of problem, the recommendations made to resolve the problem, the drug involved and the potential clinical significance of the problem. The system has been successfully used by our group in a previous study of medication reviews, as well as a study of clinical interventions made by community pharmacists. This part of the project will provide details of key aspects of HMRs that currently are not known. This will include:

the number and type of problems identified per HMR
the individual drugs/drug groups associated with problems (of different types)
the recommendation(s) made to resolve the problems, and importantly,

whether the recommendations were accepted/undertaken by the patient/GP (uptake percent).
