Methodology Outline:

Figure 1 outlines the four phases of the Valmer Project:

1	HMRs obtained: 200 pharmacists submit details of the next 5 HMRs undertaken (1000 medication reviews)

2	HMRs recorded and classified: All HMRs are examined, and drug-related problems classified by drug, type, resolution and uptake of recommendation/s

3	HMR outcomes assessed: 4 x 4 member expert panels assess 180 HMRs and provide probability estimates for different consequences at different levels of severity, with and without the HMR

4	Economic value estimated: probability estimates used to determine the value of the HMR in terms of healthcare costs and various health resource utilisation parameters
